Experienced Cardiac Nurses - an exciting opportunity to provide clinical expertise in the role of Nurse Practitioner (NP) Heart Failure Supportive Care. This role will be part of a multidisciplinary team approach to provide supportive care to patients with advance heart failure.

Where you'll be working

The NP Heart Failure Supportive Care will work within the NSLHD - MACARF Heart Failure service with the support of the RNSH Palliative Care Department and NSLHD Palliative Care Network, in collaboration with other Acute Services, Hammond Care Palliative Care services and Northern Sydney HomeNursingService.

What you'll be doing

The Nurse Practitioner (NP) Heart Failure Supportive Care will work within the NSLHD - MACARF Heart Failure service with the support of the RNSH Palliative Care Department and NSLHD Palliative Care Network, in collaboration with other Acute Services, Hammond Care Palliative Care services and Northern Sydney HomeNursingService. This is with the aim of providing expert palliative care assessments and care associated with usual heart failure management.

The NP is an expert in heart failure management with palliative care skills who will support the educational and professional development of nursing groups, allied health staff, patients, carers and their families. A core function of the role is to be a conduit for heart failure patients transitioning to end of life care and establishing links with NSHNS, Hammond Care Community Palliative Care services and Residential Aged Care services depending upon needs and patient wishes. The NP/TNP works within a defined scope of practice and operates at an advanced practice level and can independently manage a range of patient palliative/end of life care options which best meets the needs of the patients’/carers/ families.

The NP can provide comprehensive systematic patient assessment and can determine the appropriate interventions necessary to deliver advanced and holistic management of patients attending the NSLHD Heart Failure clinics or when necessary, inpatient care. The NP will refer to the Heart Failure Cardiologist or Palliative Medicine specialists for issues outside the scope of the NP.

The NP can extend nursing practice by facilitating timely patient review and manage patients that present with palliative/end of life care needs. The NP is an expert clinician who provides evidence-based autonomous clinical care for patients that are attending both NSLHD heart failure clinics and inpatient wards depending on their needs.

People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:

- Current registration with AHPRA and authorised to practice as a Nurse Practitioner by the NSW Nurses & Midwifery Registration Board (AHPRA) or working towards such a qualification. If transitional, 5 years full time equivalent post registration experience, with at least 3 years full time equivalent clinical experience in the specialty of the Nurse Practitioner role.

- Demonstrated clinical competency in advanced practice management in Cardiac Failure with an interest in Supportive and Palliative Care.

- Ability to independently assess, investigate and implement appropriate treatment for patients within advanced nursing practice guidelines.

- Demonstrated ability to work autonomously and have effective communication and negotiation skills.
