Care Coordinator - Paediatric Community Led Integrated Health Clinic

Located in Churchill, part-time, fixed term (0.8 EFT until 30 June 2026)

Work with some of the most vulnerable clients in the Latrobe Valley and have an impact on their health outcomes.

The salary for this role is $48.1763 to $55.98 per hour.

Join us as a Care Coordinator in our Paediatric & Youth Hub and be part of a passionate team that makes a real difference in the lives of children and families.

The Community Integrated Health Clinic is funded by the Gippsland Primary Health Network. The clinic is conducted in partnership with Berry Street.

The Care Coordinator works with a case load of children and families, linking them into health and wellbeing services to ensure they are not being disadvantaged. The short term intensive care coordination model aims to not only support the child and family to access the services they require, but to also build their capacity to manage their healthcare independently in the future.

The service provides a whole of family approach, working with each child and family to determine their individual goals and assisting them to achieve these.

This current vacancy is for someone with a strong background working with families from first nation, refugee and/or cultural and linguistically diverse backgrounds. The successful applicant will need to be able to demonstrate a high standard of culturally safe care.

To be successful, you will require:

- Bachelor of Nursing, Allied Health, Social Work or equivalent
- Relevant professional registration
- Current Victorian Drivers Licence
- Experience working with children and families
