JOB DESCRIPTION

We are seeking a passionate and experienced NDIS Lifestyle Support Worker to join our team in Clermont, QLD. This part-time role (can grow to full time) will involve providing personalised support services to help our clients live independently and engage with their local community. We provide the opportunity to grow your career in a supportive and dynamic environment.

What you'll be doing

- Assisting clients with daily living activities such as personal care, meal preparation, and household tasks
- Providing social and recreational support to encourage community participation and independent living
- Developing and implementing individualised support plans to meet client's goals and needs
- Maintaining detailed records and documentation in line with NDIS requirements
- Collaborating with the wider support team to ensure a coordinated and holistic approach to client care

What we're looking for

- Relevant qualifications in disability support, aged care, or a related field
- Demonstrated experience providing person-centred support services to NDIS clients
- Strong communication and interpersonal skills to build positive relationships with clients
- Commitment to upholding the rights and dignity of people with disability
- Ability to work independently as well as part of a collaborative team
- Current First Aid and CPR certificates, and a valid driver's licence
