We are currently seeking an experiencedHome Care Coordinatorto join our team. The ideal candidate is committed to best practices, communicates effectively with individuals from all backgrounds, and approaches every situation with care and compassion. - Hours:Full-time, Monday to Friday, 8:30 AM – 4:30 PM
- Reporting to:Care Manager
- Remuneration:Competitive base salary + Superannuation
- Travel Requirement: Covering Northern Rivers Region

Key Responsibilities:

- Conduct client care assessments and oversee ongoing care planning
- Maintain client compliance and manage a caseload of clients
- Coordinate and deliver personalised care aligned with client needs, care plans, and allocated budgets
- Perform general administrative tasks related to the role

Skills and Experience:

- Relevant qualifications (Cert IV or Diploma in Nursing highly regarded)
- Experience in aged care, NDIS, or mental health case management (preferred)
- Knowledge of My Aged Care, Home Care Packages, and NDIS frameworks
- Proven experience in assessing and planning community-based care
- Proficiency in Microsoft Office 365, SharePoint, Outlook, and rostering software
- Excellent verbal and written communication skills with strong customer service abilities
- Ability to handle incoming calls professionally and courteously
- Strong organisational, problem-solving, and decision-making skills
- Collaborative team player with a proactive attitude
- Ability to build strong relationships with external organisations for client referrals
- Commitment to confidentiality and privacy in line with company policies
