We are seeking a Support Coordinator to join a growing business

**Who Are We?**

Community Home Care Service is a small non-for-profit organization made up of a team of dedicated and skilled professionals who take pride in their work. We are located in the heart of Western Sydney with home care clients all over Sydney.

**The Opportunity**

We have a permanent full-time position - Monday to Friday, 9 to 5 - available for an experienced Support Coordinator to join our growing team. You will have the opportunity to support our established client base. You will be provided with your own office and a supportive admin team to help you. There is ability for growth

**As a Support Coordinator you will be**
- Developing, implementing and managing Home Care Packages
- Building and maintaining strong working relationships with a number of stake holders including the client’s family and allied health providers
- Partnering with consumers in the development and implementation of their consumed driven care plans and continue to provide them with ongoing support
- Ensuring that each client is aware of their budget and the package is managed within its budget
- Maintain accurate and up-to-date documentation in line with company’s policies and procedures

**What we need from you**
- Certificates in Age and Community Care
- At least 1 year experience in this or similar field
- A high level of competency in the English
- Ability to speak Spanish desirable but not necessary
- Computer skills with a working knowledge of Microsoft Office Suite and the ability to learn a new database
- Excellent time management and organization skills
- Eagerness to learn and stay up-to-date with changes in regulations
- Excellent communication and interpersonal skills
- Enthusiasm to work as part of a dynamic team
