Support at Home Program Support Officer - Rockhampton

We’re seeking a vibrant and people-focused Support at Home Program Support Officer to join our passionate team in Rockhampton. If you thrive on meaningful connections, love organising, and genuinely care about improving lives, this is the role for you.

Responsibilities:

- Connecting with clients, families, and health professionals
- Coordinating services and developing individualised care plans
- Supporting smooth onboarding and ongoing engagement
- Making every interaction matter

Requirements:

We’re seeking someone who is both caring and capable, with a high level of professionalism.

- Experience in care coordination, case management, or a similar role
- A background in aged care, disability support, or community services
- Excellent communication, time management, and problem-solving skills
- A strong ethical approach and attention to detail
- Confidence using technology and care management systems
- A current driver’s licence and access to a reliable vehicle

Qualifications:

- Diploma qualified nurse highly encouraged to apply, RN / EN
- Certificate IV or Diploma in Aged Care, Disability, Community Services or similar (preferred) with minimum 2 years' experience
- Knowledge of Home Care Packages (HCP) and or NDIS (Desirable)
