About the Role

AHP-2 In this meaningful and rewarding role, you’ll provide specialist psychological care to adults living with chronic heart failure as part of a dedicated multidisciplinary team. You’ll conduct psychological assessments, design and deliver tailored interventions, provide group education sessions and support patients to improve their wellbeing and self-management. Working collaboratively within a supportive multidisciplinary team, you’ll contribute to the ongoing development and evaluation of the Chronic Heart Failure Self-Management Program, to enhance patient outcomes and promote long‑term health and quality of life.

AHP-3 In this senior role, you’ll provide advanced psychological care and leadership within the CALHN Chronic Heart Failure Self‑Management Program, supporting adults living with heart failure to improve their mental health, wellbeing and self‑management. You’ll deliver individual and group interventions, guide evidence‑based practice, and contribute to service planning, evaluation and research. As a clinical leader, you will provide supervision to provisionally registered Psychologists on clinical placement and support other psychologists within the CALHN psychology team, fostering a culture of learning and excellence in psychological care.
