TheRole
As a therapist at Connect2Care, you will be joining amultidisciplinary team. You will provide evidence-based services to peoplethrough multiple service delivery models, including telehealth, communityvisits and centre-based services. Referrals may come through the NDIS, Medicareor privately. It is a mobile role, with the option to work from home or ourhubs when not seeing clients. The role will involve the following:
- Assessing and providing interventions forchildren & adults living with a range of disabilities
- Working within the NDIS to maximise clients'level of function, capacity & social engagement
- Assisting clients with assistive technologyassessment, recommendations and applications (in relevant fields)
- Timely documentation & report writing
- Managing a varied caseload
- Meeting company professional expectations (e.g.meeting billables & effective client management)
- Building relationships with key stakeholders,including therapists, clients, & referral sources
- Working across leading software platforms &online tools
The NewGrad Program
Connect2Care has developed and specifically tailored a NewGraduate Program to make the transition smoother for new graduates entering theworkplace. The program fully supports recent graduates, enabling them toacquire the skills needed to launch them into a successful career. Spanningover 12 months and starting from the moment you step through our doors.

- Fortnightly group mentoring sessions

Requirements
To apply for the role, you will need to have or be able toobtain the following:
- Relevant tertiary qualifications in relatedfield
- Discipline-specific professional registration(e.g. AHPRA/SPA)
- Working with Children Check (WWCC or Blue Card)
- NDIS Worker Screening Check
- Police Check & International Police Check(only applicable to individuals who have lived overseas)
- Drivers licence & access to a reliablevehicle
- Car insurance
- Professional Indemnity & Public Liabilityinsurance
