Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute, sub-acute, aged care and community services.
An exciting opportunity exists for an Older Person's Nurse Practitioner to join our community team in the provision of clinical care within our Residential In Reach service. This role will also require support to our Transition Care Program. This position will provide clinical leadership within the multi-disciplinary team and work within credentialed scope of practice to undertake comprehensive assessment, diagnosis and clinical intervention. The successful applicant will be supported by a Staff Specialist Geriatrician and will work alongside an existing Older Person's Nurse Practitioner to grow our NP service.

The successful applicant will benefit from
