Opportunityfor a registered Practice Nurse with Primary Care experience to become a memberof our excellent clinical team here at Seven Dials Medical Centre.Responsible for the day to day running of the treatment room, organising yourclinics and ensuring all patients receive the best possible nursing care.

You will be focusing on preventative care, health promotion and management of long term conditions for patients across all ages and cohorts

Main duties of the job

Providing assessment, screening treatment services and health education advice:

- Dietary advice
- Urinalysis
- Health screening
- Dressings
- ECG
- Contraception
- Routine immunisations and vaccinations
- Removal of sutures / clips
- Baby immunisations
- Assisting with the fitting of IUCD / IUS
- First registration checks / health surveillance / general health advice
- Organise and coordinate the provision of nursing services for the practice to include the delegation of duties to other members of the practice nurse team as appropriate
- Provide general and specific health screenings to the practice patients (within agreed protocols) with referral to other members of the primary and secondary health care team as necessary

SevenDials Medical Centre proudly serves just over 7,000 patients in Brighton andHove and has 14 members of staff including 3 GP Partners, an ACP, PracticeNurse and HCA. We work closely with the PCN to offer patients access to SocialPrescribers, Mental Health Co-ordinators, Care Co-ordinators and a Pharmacyteam.

We work hard to deliver the best levels of care to our patientsand pride ourselves on a 'Family GP' feel. Many of our patients have been withthe surgery for decades and we regularly receive positive feedback frompatients both long standing and newly registered.

Job responsibilities

Implement and evaluate individual treatment plans for chronic diseasepatients

Identify, manage and support patients as risk of developing long-termconditions, preventing adverse effects on the patients health

Provide routine nursing care to patients as required in accordance withclinical based evidence, NICE and the NSF

Provide wound care (ulcer /Doppler etc.) to patients

Provide chronic disease clinics, delivering patient care as necessary,referring patients to secondary / specialist care as required

Maintain accurate clinical records in conjunction with extantlegislation

Maintain chronic disease registers

Recognise, assess and refer patients presenting with mental health needs

Implement vaccination programmes for adults and children

Support the clinical team with all safeguarding matters, in accordancewith local and national policies

Person Specification

Knowledge and Skills

- Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
- Broad knowledge of general practice and experience of working as a Practice or Community Nurse
- Experience with chronic disease management
- Experience of working autonomously
- Experience of infection prevention and control measures
- Experience of managing elderly and housebound patients
- Experience of Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and womans health (cervical cytology, contraception etc.)
- Request pathology tests and process the results, advising patients accordingly
- Understanding of the importance of evidence-based practice
- Ability to promote best practice regarding nursing matters
- Ability to work within own scope of practice and understanding when to refer to GPs
- Good clinical system IT knowledge and an ability to record accurate clinical notes
- Ability to communicate complex and sensitive information effectively with people by telephone, email and face to face
- Understanding of safeguarding adults and children
- Understanding and knowledge of healthcare provision in GP surgeries, QOF and enhanced service
- Effectively able to communicate and understand the needs of the patient
- Effective time management (planning and organising)
- Ability to follow legal, ethical and professional policies/local policies/procedures and codes of conduct
- Understand the requirement for PGDs and associated policy
- Ability to use own initiative, discretion, and sensitivity
- Able to get along with people from all backgrounds and communities, respecting lifestyles and diversity

- Knowledge of national standards that inform general practice (NSF NICE guidelines)
- Demonstrate personal accountability, emotional resilience and work well under pressure
- Knowledge of IT systems including the ability to use word processing skills, emails and the internet
- Access to own transport and ability to travel across the locality

Qualifications

- Registered Nurse (Nursing and Midwifery Council)
- Has evidence of working at the relevant level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework
