**About the role**

Southern Cross Cares Community Wellness and Lifestyle Services is seeking Occupational Therapy, Physiotherapy, Exercise Physiology, Social Work, and Exercise Science graduates passionate about health and wellness for older adults, and looking to work in a dynamic, growing program. Applicants must be willing to work in a person-centred, multi-disciplinary environment that embraces innovative practice and focuses on the reablement of our clients.

Southern Cross Care’s Graduate Program has been developed to provide formal and informal mentoring and support to new graduates entering the allied health workforce. Successful applicants will undertake their own caseload of clients as well experience cross and interdisciplinary learnings, a variety of supported tasks, including a group project, that will increase and enhance their future opportunities and broaden their understanding of the aged care sector as a whole.

**The key roles of the position are**:

- Providing a person centred safe and effective service that provides opportunities for clients to experience and enhance their physical, mental and social wellbeing
- Work positively and collaboratively with other staff and qualified practitioners to provide proactive person centred service and support
- Individual and/or group exercise programs that are tailored to meet each client’s needs, goals and preferences
- Work collaboratively within the multi-disciplinary team to ensure clients are on their best health pathway
- Demonstrate innovation in service delivery
- Opportunity to work across both Community and residential sites

**About you**

You will hold a relevant degree and be eligible for registration with your professional body (AHPRA / ESSA). In addition you will demonstrate exceptional listening, communication, and time management skills, and a willingness to drive innovative practice.
