The Northern Adelaide Local Health Network (NALHN) provides care for more than 400,000 people living in the northern metropolitan area of Adelaide and boasts newly redeveloped wards and Emergency and Outpatient Departments, with plans for the addition of more brand-new facilities in the near future. NALHN provides a full range of high-quality medical, surgical, maternity, diagnostic, emergency and support services. NALHN offers a range of primary health care services across the northern metropolitan area of Adelaide, with a focus on providing preventive and health promoting programs in the community, and transition and hospital substitution and avoidance programs targeted at chronic disease and frail aged. Our 400-plus Allied Health professionals play an integral role in multidisciplinary teams delivering exceptional care to patients across our acute, subacute and hospital avoidance services.

- The Allied Health Practitioner - Chronic Disease Self-Management is operationally responsible to the Senior Manager, Intermediate Care Services for the development, implementation, coordination and evaluation of quality Allied Health Chronic Disease Intermediate Care Services (within a self-management support framework) for patients of the Northern Adelaide Local Health Network (NALHN).- The Allied Health Chronic Disease Intermediate Care Services bring together Dietetic, Exercise Physiology, Occupational Therapy, Podiatry, Physiotherapy, Social Work and Case Management services working collaboratively with services from the NALHN Clinical Divisions and Aboriginal Health Services to optimise health outcomes and reduce hospital admissions for patients with Diabetes, Cardiac and Respiratory Diseases, and vulnerable people with other chronic diseases.-
- Working in conjunction with other Chronic Disease Intermediate Care Services members and other health professionals the Clinician optimises patient functional and health outcomes in this specialty area.To be eligible for this position you must hold an appropriate degree or equivalent qualification for one of the following Allied Health disciplines working in the service, specifically:

- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Dietitians Association of Australia (DAA).
- Appropriate Degree or equivalent qualification, usually in human movements, inclusive of the exercise physiology elective and eligible for accreditation from Exercise Sports Science Australia (ESSA).
- Appropriate Degree or equivalent qualification which entitles registration as an Occupational Therapist with the Occupational Therapy Board of Australia.
- Appropriate Degree or equivalent qualification which entitles registration with the Physiotherapy Board of Australia.
- Appropriate Degree or equivalent qualification which entitles registration with the Podiatry Board of Australia.
- Appropriate Degree or equivalent qualification recognised by the PsyBA and general registration with PsyBA.
- Appropriate Degree or equivalent qualification which gives eligibility for full membership of the Australian Association of Social Workers.

COVID-19 vaccination is an essential requirement under the Emergency Management (Healthcare Setting Workers Vaccination) (COVID-Vaccine and screening requirements are set out in Addressing vaccine preventable disease: Occupational assessment, screening and vaccination policy directive (see Mandatory Instruction 1 in the policy).
