**Who are we looking for?**:
We are seeking a driven and enthusiastic Allied Health Assistant who is passionate about making a positive impact on the well-being and lives of older people. Under the direction of a multidisciplinary team, you will provide support to customers in our Transition Care Program (TCP) team. The program provides rehabilitation and engagement for people returning to their homes after a hospital stay. This is an integral role which empowers our customers to recover in their own homes and ensures they will receive appropriate care.

We would love to hear from you if you have a Cert III or IV in Allied Health Assistance or have equivalent experience and background working with vulnerable people or as part of a multi-disciplinary team.

**What can you expect to be doing?**:

- Collaborate with therapists to deliver therapy programs.
- Providing support and services in customer homes.
- Support and supervise customers with daily living activities.
- Work with customers towards achieving health goals and positive outcomes.
- Assist with the coordination of services, such as appointment bookings and notifications and in-clinic tasks related to clinical care.

**What do you need to bring?**:

- Certificate III or IV in Allied Health Assistance or Cert IV in Fitness _(essential)._
- Minimum one year experience in a similar role _(essential). _
- Outstanding interpersonal and communication skills, both written and verbal.
- Excellent time management and organisational skills.
- Ability to work effectively in a multidisciplinary team environment.
- A current Australian driver's license and roadworthy vehicle _(essential). _
- Current or willingness to obtain annual flu vaccination and double dose of COVID-19 vaccination _(essential). _
