**The Role**

The Social Worker will provide case management to young women and their children living in in our outreach program. In line with the Young Family Support Program service model and approved Case Management Guidelines the Social Worker will provide case management support including developing an individualised support plan with their clients. The individualised support plan may cover issues such as housing, parenting, independent living skills, mental health and wellbeing, substance misuse related issues, social and recreational supports.

The Social Worker will work with the young parent to develop a support plan for their child, being mindful of the impact of homelessness on children’s health, attachment and development.

**To be successful in this role you will need**:

- Tertiary qualifications in the Human Services Field
- Ability to achieve effective client outcomes through quality case management
- Experience working with vulnerable young people with complex needs and an understanding of the impact of childhood trauma, attachment theory and adolescent development
- Knowledge of the homelessness sector and intersecting services and agencies
- Proficient in Microsoft Office
- Strong written and verbal communication skills
- Valid Department of Human Services (DHS) Working with Children Check
- Ability to undertake some work out of standard work hours when required
- Valid Australian Drivers Licence

**Desirable**
- Degree in Social Work, Psychology, Counselling or related field.
- Experience in using H2H data collection system
- Experience working with young parents and/or young people.
