**A bit about the role**

In the position of **Community Connector**, you will be reporting to Team Leader Aging Well Community Partnerships and will be working closely with a team of Community Connectors, Neighbourhood Hubs and contributing to the Community, Connections and Care Service Unit.

Your role is to connect our residents, their carers and families to locally based services to re-enable and enhance their wellbeing and independence. Forming partnerships and collaborating internally within Wyndham Council and external agencies.

**What you will deliver**
- Short term support for people living in Wyndham, their carers and family to engage in services and supports of their choice that meet their needs.
- Consistent delivery of support to the community with the department’s integrated service delivery principles, a place-based approach and the principles of family centred practice.
- Engaging collaborative partnerships with Council departments, local place-based service providers and other organisations to understand and promote what is available, identify gaps in the system and develop initiatives to meet the gaps.
- A people-centred approach that enhances and maintains the abilities and independence of individuals

**What you will bring**
- Completion of a degree or diploma qualification in Community Services or related field with some relevant experience, or, lesser qualifications and substantial relevant experience, or, specialist experience in the field.
- Highly developed customer service skills with respect and understanding of diversity and difference in the abilities of others.
- Demonstrated skills and knowledge working in Community Care that aims to enhance an individual’s independence
