**A bit about the role**

Our Community Care team support people under 65 years and Aboriginals or Torres Strait Islander people aged under 50 years who require assistance with activities of daily living. This may be due to physical and/or psychosocial functional impairment related to disability (for which they are not eligible for the NDIS), chronic illness and short-term health needs.

Working independently in client’s homes and in the community, you will provide flexible support services to a diverse range of people and their carers using an individualised approach to identify their strengths and build capacity to achieve their goals and enhance their independence and wellbeing, such as:

- Domestic Assistance
- Meal Preparation
- Personal Care
- Shopping (escorted and unescorted)
- Respite, and
- Social Support Groups
- **_Domestic Assistance is our highest demand services._**

The number of shifts and hours will depend on your availability and demand for service.
- Allowance to reimburse travel time between clients
- Kilometres reimbursement, and
- Mobile phone and laptop provided.

**What you’ll deliver**
- A person-centred approach that enhances and maintains the abilities and independence of individuals.
- Engage with clients, carers and/or family members to flexibly support an individual’s goal attainment.
- Strong relationships with clients, their carers and/or family and other support networks, embracing a collaborative approach to your work.
- Innovative and creative solutions to accommodate changing circumstances.

**What you’ll bring**
- Certificate III in Individual Support, Aged Care or equivalent
- Knowledge of issues relating to disability and ageing
- Strong communication skills for building trust and rapport with clients
- Demonstrated skills and knowledge working in Community Care
- Current employee Working with Children Check
- Current Victorian driver’s license
- Innovative, positive, and flexible mindset
