A bit about the role


Wyndham's Community Connections and Care Unit provides quality services, information, advocacy and support for older people and those with a disability or health conditions.


Some of the people we support are people who are under 65 years and Aboriginals or Torres Strait Islander people aged under 50 years who require assistance with activities of daily living.


This may be due to physical and/or psychosocial functional impairment related to disability (for which they are not eligible for the NDIS), chronic illness and short-term health needs.

Working independently in client's homes and at community venues, you will provide flexible support services to a diverse range of people and their carers which enable, re-enable and enhance their wellbeing and independence, such as:

  • Domestic Assistance
  • Personal Care
  • Respite, and
  • Social Support


You will enjoy working directly with the community and your Team Leader, to meet the program objectives, you will have excellent time management skills with flexible availability throughout the week.


What you'll deliver

  • A personcentred approach that enhances and maintains the abilities and independence of individuals
  • Engage with clients, carers and/or family members to flexibly support an individual's goal attainment
  • Strong relationships with clients, their carers and/or family and other support networks, embracing a collaborative approach to your work
  • Innovative and creative solutions to accommodate changing circumstances

What you'll bring

  • Strong communication skills for building trust and rapport with clients
  • Demonstrated skills and knowledge working in Community Care that aims to enhance an individual's independence
  • Knowledge of issues relating to disability and ageing
  • Certificate III in Individual Support, Aged Care or equivalent
  • Current driver's license
  • Innovative, positive, and flexible mindset
