**Overview**

The Home-Based Services (HBS) Coordinator is responsible for the coordination of personal care, social support and home help services for frail, aged people within a regional community to promote the individual’s health and well-being and enable them to remain living independently in their own homes within the relevant funding, standards & guidelines. Developing strong links with clients, families, professional staff and other home support agencies in the region is a key requirement of the role.

HBS Coordinator is also responsible for coordination of daily operations including and not limited to: prioritise referrals, have oversight of home based assessments and reviews, develop service plans, consumer scheduling and staff rostering, supervision of home support workers, monitoring waiting lists and payroll processing. HBS Coordinator supports the HBS Clinical lead and reports to the HBS Team Leader.
