Overview


We are looking for a dedicated Enrolled Nurse with a passion for supporting aged people's rights to safe and effective care in their own homes.

As an Enrolled Nurse you support the Registered Nurse/Midwife in providing person-centred care consistent with regulatory and statutory requirements.

As the Home Care Package Co-ordinator, you will oversee the provision of Home Care Package Services within the Yorke and Northern Region; planning and co-ordinating services including those of other disciplines or agencies as required to meet individual and/or group health care needs.


Duties may include monitoring client are plans and participating in clinical auditing and/or evaluative research to ensure appropriate client care outcomes are achieved; working within and promoting a model of patient/client centred care and support for client's self determination in life processes; ensuring the efficient and effective utilisation of brokerage and contractual obligations of funding bodies and advocating for the provision of appropriate and responsive services for clients.

In this role, you will be working under limited direction with flexible working conditions.

You will be advocates for consumer choice to direct their own life choices in a non-judgemental partnership approach to health care.

Frequent travel within the Yorke and Northern Local Health Network expected.
