**About You**
- You have demonstrated ability to identify appropriate community based services and supports (including Residential Care) for Aged Care Clients and facilitate access to these as required.
- You have a degree in Social Work with eligibility to join the AASW or Degree in Occupational Therapy or Physiotherapy with AHPRA registration, Registered Nurse - Division 1 Registered Nurse (APHRA registered).

**What you'll be doing**

The Aged Care Assessment Team (ACAT) is a multidisciplinary team providing assessment services to clients in both the community and the hospital setting. The team also assists with the formulation of care plans to meet the needs of the client and their carer, according to Department of Health, My Aged Care Assessment Manual.

The ACAT Assessor role is responsible for the provision of safe, compassionate, person-centred care through the comprehensive assessment of care and support needs of older people and their carer's. Applications welcome from clinicians with good aged care knowledge, excellent report writing skills and a passion for working with older persons and who want to be part of a friendly team of Registered nurses, Occupational therapists, Physiotherapists and Social workers.

Applicants need to have a strong commitment to working to a high standard within very tight timeframes. ACAT assessors work from various locations across the Central Coast community sites and provide assessments in hospitals, residential care facilities and in the community.
