**About the Role**

As a Team Leader within our aged care program, you will be providing support to clients who access our Home Care Packages, Transitionary Aged Care Program and Veterans Home Care Packages, in Southern New South Wales. You will work in partnership with our clients, support workers and various stakeholders to ensure clients have up-to-date care plans reflecting their individual goals and access to services and resources that provide them with holistic care. You will be joining a supportive and engaging team that enjoy working collaboratively.

This is a Part-Time 6 Month Maximum Term role, working 3 days per week with flexible location within Southern NSW. Hybrid working arrangement with flexibility to work from home and travel to local offices and meet with clients and relevant stakeholders as required.

**Key Responsibilities**
- Undertake comprehensive consumer assessments, care planning and review
- Develop strategies and/or broker services to address the client needs in consultation with families and communities
- Allocate home care workers and maintain changes
- Maintain regular communication with consumers and their families
- Liaise with community-based agencies and service providers to ensure an integrated and cohesive approach to service delivery

**Skills & Experience**
- Degree or equivalent experience in human services, nursing or related field
- Previous Case Management experience and a passion for working within the Aged Care field
- Current Drivers Licence
- Demonstrated experience of working with older people, including people from culturally and linguistically diverse backgrounds, and understanding of issues facing people with chronic and complex needs.
- Demonstrated ability to work in a flexible and innovative way in responding to consumer need.
- Demonstrated experience in all aspects of case management including comprehensive assessment, planning, provision and evaluation of services for older people, people with dementia and or mental health concerns and their carers and families.
