**About the facility**
- Our major sites include University Hospital Geelong in Geelong’s CBD, the McKellar Centre precinct in North Geelong, Barwon Health North in Norlane, and residential aged care facilities spanning the region. Comprehensive mental health, community health and dental services are delivered through various programs across the Barwon South West Region, including Torquay and Colac.

**About the role**
Working within a multi-disciplinary team environment (telehealth / face to face) this position focuses on the provision of government funded carer support services that are responsive to the needs of the local community. This position encompasses a variety of roles including information provision; guided referrals; facilitation of peer support groups; community outreach; developing carer capacity and resilience; delivering practical and innovative practical support. In this role you will:

- Undertake holistic assessments of clients' need and develop goal-directed care plans
- Respond to emergency, practical and emotional needs of carers
- Coordinate service delivery and refer to other service types as required
- Facilitate peer support groups
- Contribute skills, knowledge and expertise to promote community awareness and understanding of carer issues though outreach and promotional activities
- Providing carer coaching and support to carers to reach their goals
