**The Opportunity**

The Homeless and at Risk of Homelessness Response (HaRH) program focuses on the Brimbank, Melton and Hobsons Bay LGAs to support those who are experiencing homelessness, at risk of homelessness or living in high-risk accommodation including residents in rooming houses and caravan parks. Our multidisciplinary team, through a coordinated, collaborative client centred approach provides targeted and tailored engagement strategies, undertake wholistic health and wellbeing assessments, facilitates and supports appropriate active linkages into IPC Health, committed priority access, services and/or referrals into external services, support and enhance the client journey to better health outcomes.

As a Community Engagement Officer you will use your proven and substantial community engagement, relationship building and networking skills and experience to support and enhance the health and wellbeing journey of people experiencing homelessness or at risk of homelessness and those living in high-risk accommodation.

You will be well supported by the Manager Community Connections and be part of a passionate team working closely with our Wellbeing Coordinators collaborating to develop and build this meaningful service for the community.

Based at Deer Park, the opportunity is a 0.8FTE Part Time Ongoing role (4 days per week) and is classified as a Social and Community Services Employee Level 4 under the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi Enterprise Agreement 2022. This role is offering a remuneration of $85,120 pa (Pro Rata Part Time) + superannuation.

**You will make a difference by**
- Leading the development and implementation of assertive outreach engagement strategies supporting target groups living in Brimbank, Melton and Hobsons Bay LGAs.
- Provide meaningful engagement and case planning, including identification and initial assessment of issues impacting on target groups’ health, wellbeing and housing.
- Identifying and facilitating appropriate referral pathways, internal to the HaRH program, IPC Health and local external support services for clients presenting complex needs.
- Contribute to the development, implementation and maintenance of data collection, recording and reporting.
- Identify, develop and maintain key stakeholder and project partner relationships.
- Commitment to enhancing the service model and achieve the best client outcomes through continuous improvement approach.

**To succeed you will need**:

- Qualification in community development, community engagement, social work or related area with a minimum of two year’s experience.
- Proven experience and knowledge of community engagement strategies to effectively engage with individuals and communities experiencing complex health, wellbeing, homelessness and housing issues.
- Experience in a broad range of stakeholder engagement including community health, wellbeing and homeless sectors.
- Demonstrated experience in client data collection, recording, analysis and reporting.
- Proven ability to meet key performance target.
- Strong technical skills including Microsoft Office suite, client management systems (Trak Care advantageous), communication, written and reporting skills.
- A current and valid Victorian Working with Children's Check and drivers licence.
