About the role
We are seeking a dedicated Homelessness Case Manager You will provide intensive case management support and outreach to people who are homeless or at risk of homelessness in order to ensure they have access to affordable, safe accommodation and prevent their return to the homelessness service system.
What is the Gateways Program?
The Salvation Army Gateways program is a crisis accommodation residential program catered to providing intensive case management support to clients whilst addressing presenting needs, risks and vulnerabilities. This program supports single persons 16 and over, families of any age with accompanying children who are homeless or at risk of homelessness either within the crisis residential setting, crisis properties within the community and community outreach.
Responsibilities include
- Client referrals are assessed in accordance with program criteria
- Clients receive multiple weekly contacts with safety/risk plans developed
- Individual case, safety and or private rental action plans are developed in collaboration with clients and include a range of interventions
- Clients assisted to access support services and establish community / specialist supports
- Case notes are to be completed within 24 hours of each client contact and documents uploaded to TSA information systems
- Ensure clients are provided with information pertaining to The Salvation Army and our commitment to the safety and inclusion of LGBTIQA+ people
This is a permanent part-time position, 30.4 hours per week, based in Croydon, VIC. Working hours are 9am-5pm however, 1 day per week, working hours will be 11:30am-7:36pm. Salary and conditions are in accordance with SCHADS Level 5 Award.
You will have
- Tertiary qualification in Social Work, Social Welfare or related field
- Demonstrated experience accompanying highly developed skills working with persons experiencing homelessness who display a range of highly challenging behaviours (essential)
- Knowledge of service systems, in particular youth, housing and homelessness, Child Protection, AOD, Family Services and Mental Health
- Demonstrated skills in engaging homeless persons
- Willingness to assist in provision of support in areas of domestic violence, drug & alcohol, mental health, living skills and parenting
- A valid Employee Working with Children's Check
- A current Victorian Driver's Licence
