This privately owned Retirement Living and Aged Care provider has various locations across Queensland. They provide experienced, flexible, and innovative care solutions to ensure client's choice and dignity come first as well as meeting current and future care needs.

**About the Role**:
This role combines Leadership Skills, Management Experience, and Aged Care Knowledge into one. You will report to the Community manager and work alongside the Clinical Team to continuously improve the Clinical Outcomes of the residents.

You’ll be a great communicator who can cooperate with a diverse range of clinical professionals, have strong clinical abilities so you can coordinate the best treatment plans, and be eager to continuously improve your skills so you can lead to the best of your abilities. Some of your responsibilities will include:

- Collaborate with Community Manager on major issues related to service provision
- Oversee Care Coordinators who consult with residents to design, monitor, review, and coordinate care plans following comprehensive care assessments
- Consult with Registered Nurse on care matters; engaging clinical nursing expertise to assess, plan, monitor and review complex clinical care elements as required
- Successfully recruit and onboard care staff, ensuring a positive employee experience
- Oversee team to ensure coordinate service interface with external clinical professionals (General Practitioners, Medical Specialist, Hospitals, Nurses, and Allied Health)
- Monitor training and education of staff to ensure development of skills and compliance
- Rostering of staff and scheduling of resident care services to meet required care needs
- Maintain documentation consistent with all legislation and quality standards
- Manage, review, and investigate all care related incidents, putting appropriate actions in place and ensuring senior management are informed of reportable SIRS
- Hold case conference with residents/representatives as required
- This role is Monday to Friday, however from time to time you may need to support a 7-day business, taking after-hour calls and/or supporting critical situations

**Required Skills & Experience**
- Current registration with the Australian Health Practitioner Regulation Agency (APHRA) as a Registered Nurse
- 3 to 5yrs+ experience HCP management and/or clinical role managing teams
- Demonstrated knowledge of the DVA Community Nursing Program, Home Care Packages, Aged Care Quality Standards and Client Rights and Responsibilities
- A strong leader who collaborates, trains, and supports employees
- Empathy for and a desire to work with the aged and their families
- Demonstrated background in in residential and/or home and community care
- Advanced communication, interpersonal, problem solving and organisational skills
- Strong computer, interpersonal, problem solving and time management skills.
- A commitment to consumer-centred care and the well-being of the consumer
- Current Police check
- Covid-19 vaccine evidence
- Current Drivers Licence
- Rights to work in Australia
