**About the role**

The ACAT Assessor position stands within the of the CCH/ACAT multidisciplinary team. The primary purpose of the position is to provide holistic, comprehensive assessment to clients referred for the Transitional Aged Care Program and to the target population in the St George Geographical area in the community, hospitals, and aged care facilities as applicable.

**Your Key Responsibilities include**:

- Comply with the CHCK documentation standards and discipline specific guidelines for any entries into the electronic medical record with 100% of medico-legal requirements for documentation met.
- Participate in QI activity within the department as directed
- Meet and maintain record of CNS professional requirements
- Maintain ACAT KPI'S

**What You Bring**
- Registered Nurse with current registration with the Australian Health Practitioners Regulation Agency (AHPRA), plus relevant post Graduate Qualifications
- Minimum of 3 years post graduate experience working with older people in an acute, aged care and/or community setting and current unrestricted Class C driver’s licence
- Knowledge of Aged Care Systems as well as Aged Care issues and experience working with clients from diverse cultural backgrounds.
- Demonstrated ability to provide education to clients, peers, and carers as well as commitment to ongoing self-education and best practice.
- Demonstrated high level of interpersonal, written, and verbal communication skills including computer literacy
- Demonstrated ability to work autonomously and as part of a multidisciplinary team
- Demonstrated knowledge of Quality Improvement initiatives and work within the Work Health Safety principles relative to community service setting
- Ability to work within the Mission and Values of Calvary Health Care Kogarah
