**About the role**

Currently, Direct Care Resources have exciting opportunities for experienced Case Managers to join our team on a **permanent, full-time** basis in the Disability and Child Protection sector. If you are resilient, reliable, respectful, passionate, positive, and professional this could be for you.

The role forms an integral part of a multidisciplinary Case Management team and are responsible for managing a client caseload and providing professional clinical and associated care management advice and support to the Principals and other members of the organisation to ensure that the design, implementation, management and evaluation of clinic and care practices across assigned DCR care facilities reflect recognised and relevant professional clinical care standards.

We currently service clients in Toowoomba, Ipswich, Gold Coast, Sunshine Coast and Brisbane's North, South, and West regions.

**Key Responsibilities**
- Provide professional advice and support to the Executive Manager (Care Policy and Practice) and the Principals, particularly when undertaking the development, write-up, implementation and ongoing management of Incident Reports, Individual Personal Plans (IPPs) for assigned clients with a disability and for children and young people under Child Protection Orders.
- Develop in association with other health professionals, care treatment regimens for clients with medical, mental health and/or complex developmental and behavioural issues.
- Design and deliver training and development programs for other Case Managers, DCR care staff and other relevant parties in professional care management practices associated with DCR's primary client groups.
- Conduct regular supervision and training of DCR care staff on-site and prepare appropriate reports and documentation.
- Participate, on regular rotation, in an ‘emergency after hours' contact roster to provide professional advice to DCR care staff in managing critical incidents affecting clients and client care.
- Regularly visit DCR care facilities to directly supervise, counsel and train care staff and to ensure compliance with relevant legislative and regulatory requirements including reporting, policies, procedures and practices and Quality Assurance standards relating to the care of clients, accommodation, documentation, and records.

**Requirements**:

- Hold a **Bachelor Degree** (or higher) in Social Work, Social Science, Psychology, Nursing, Human Services or similar
- Experience in casework practices with a variety of clients that have different needs and goals
- Demonstrated ability to interact positively and constructive with clients, their families, case managers, executive management, and representatives of government agencies
- Demonstrated knowledge of legislation, policy and practice in areas including disability care, forensic disability, child protection, aged care, youth justice and/or health care.
- Demonstrated high-level computer literacy, especially in Microsoft Office and the ability to use alternate between different systems.
- Demonstrated ability to complete high-level reports in a timely manger
- Ability to adhere to client's privacy and confidentiality
- Current driver's license
- Willingness to travel to support client's need
- Blue Card and NDIS Worker Screening Check or willing to obtain
- Current First Aid and CPR certificates or willing to obtain
