**Overview**

The **Women’s & Children’s Health Network** provides state of the art health care for women, babies, children and young people across South Australia. We are the state’s leading provider of specialist care for children with acute and chronic conditions as well as being South Australia’s largest maternity and obstetric service, and statewide Maternal Fetal Medicine service.

Our community-based services also reach hundreds of thousands of babies, children and young people, their families and communities across South Australia through our various services and programs in areas such as our Mental Health, Child & Family Health services, Youth Health and Aboriginal Health services.

The **Paediatric Palliative Care Service (PPCS) **provides support to families caring for a child with a life-limiting illness. This is tailored to each family’s individual needs to help make decisions about care. The Paediatric Palliative Care Service is uniquely responsible for the provision of care across the whole state of South Australia and into Northern Territory, Broken Hill and Mildura.

The PPCS is a statewide service providing specialist care for children with life-limiting conditions. Our mission is to provide excellent service to children and families facing life-limiting illness through partnership with families, interdisciplinary teamwork, and offering holistic care.

The service is a multidisciplinary team of health care specialists including:

- Doctors
- Nurses
- Grief and Loss Coordinators
- Social Workers
- Art Therapists

Join our team and contribute to providing South Australia’s women, babies, children and young people with the best health care through both within our hospital or via our many and varied community services and programs.

**The Opportunity**

An exciting opportunity exists within the **Paediatric Palliative Care Service** team to join as a **Social Worker (AHP2)** working in a **part-time** capacity, on a **12-month** contract.

In this role, you will:

- Provide clinical and consultative services across a range of service settings, including one-on-one and group evidence based psychosocial interventions for families under the care of the PPCS
- Initiate and participate in the planning, promotion and maintenance of research and ongoing planning and evaluation of the PPCS
- Ensure social work practices and protocols relating to Palliative care meet established methodologies
- Provide care across the patient journey from diagnosis and into bereavement as part of an interdisciplinary approach to paediatric palliative care
