Sub Acute Private Hospital specialising in rehabilitation, geriatric, palliative & mental health servcies through both inpatient care and day programs.

**About you**
- 5 years post graduate nursing experience
- Previous experience caring for patients with Co-morbidities
- Ability to work independently and as part of a cohesive team
- Liaise with all health care personnel for the provision of services to meet patient discharge needs.
- Resourceful & efficient in providing patient with education & access to community services
