About you

  • 5 years post graduate nursing experience
  • Previous experience caring for patients with Comorbidities
  • Ability to work independently and as part of a cohesive team
  • Liaise with all health care personnel for the provision of services to meet patient discharge needs.
  • Resourceful & efficient in providing patient with education & access to community services
