An exciting opportunity exists for a Discharge Planner to join our dynamic team at Sunnybank Private Hospital.

**As Discharge Planner your responsibilities will include**:

- Provide comprehensive patient care as part of the discharge process, including facilitation of communication with patients and carer’s and early escalation of identified issues utilizing a risk management approach
- Collaborate with the multi-disciplinary health care team, coordinate the delivery of discharge services to ensure they are patient centered, adhere to best practice and facilitate safe and timely transfer of care with the goal of reducing adverse effects and preventable readmissions.
- Develop networks with community services, professional and support groups including community referral options and provision of alternatives to inpatient care to provide the ongoing supportive care needs of patients/clients discharging improving quality of life by ensuring continuity of care.
- Early identification of issues / concerns affecting the early discharge of patients to the nursing and medical management teams.
- Support families and patients in the process of discharging from hospital to a residential aged care facility.

**Selection Criteria**:

- Demonstrated effective communication, interpersonal and team building skills
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Commitment to personal and professional development
- Good Interpersonal and time management skills with an ability to work within a multidisciplinary team
- Demonstrated ability to deliver high quality patient care
- Supports and facilitates the patient and family to make informed decisions by providing appropriate information and options regarding health and nursing care
