**Join our Psychosis Team and contribute to the mental health and well-being of your local community.**

**About the Role**:
As a Social Worker in the psychosis team, you will play a pivotal role in the clinical care coordination of our consumers. Working under the guidance of the allocated Consultant Psychiatrist or delegate, you will be responsible for implementing and delivering a range of clinical activities, including mental health and psychosocial assessments, risk assessments, treatment and care planning, therapeutic interventions, advocacy, and liaison with other community service providers.

**Key** **Responsibilities**:

- Implementing the Treatment and Care Plan developed by the multidisciplinary team and the consumer, under the guidance of the Consultant Psychiatrist or delegate.
- Providing discipline-specific assessments and interventions, utilising your specialised expertise.
- Collaborating closely with consumers and their families/carers, as well as liaising with a network of government and non-government organizations to maximise well-being, assist in stabilising mental health, support rehabilitation, and build functional capacity for self-management.

To be **successful** in this role, you will have:

- Demonstrated experience in the field of mental health and clinical care coordination.
- Strong understanding of legal frameworks and ethical practice within the field of social work.
