Central Coast Local Health District (CCLHD) is seeking an experienced Social worker to join our team in the Mental Health department.

**About You**
- You have extensive experience working with consumers with a lived experience of mental illness and experience in the delivery of a range of evidence based mental health interventions, including mental health assessment, in an individual and group context.
- You have very well developed and effective clinical communication through the use of information technology and excellent written, verbal and interpersonal skills.
- You have a Qualification of a Degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number), a demonstrated understanding of the current AASW Practice Standards as they relate to the position and a commitment to maintaining a professional skill and knowledge base.

**What you'll be doing**
- You will be in a front line, clinical position, responsible for providing support to consumers of the Mental Health Services and their families and carers.
- You will provide specialist mental health assessment, counselling and to undertake treatment and case planning, liaison and advocacy and referral to ensure access to appropriate community services.
- The role will involve patient advocacy, crisis intervention, and engagement in case management activities to ensure continuity of care and facilitate independence and recovery.
- You will work collaboratively with a multi-disciplinary team to assess and address the psychosocial aspects of patient care, develop treatment plans, provide short term counselling and therapy, facilitate access to community resources and assist with linking consumers with care systems to facilitate effective care and minimise relapse and re-admission.
