The Pregnancy Connect Care Coordinator is responsible for facilitating the planning, implementation and reporting to ensure integration of consultation, referral, and transfer pathways across local health districts and within and across relevant Tiered Perinatal Network/s.

**What you will be doing**

The Pregnancy Connect Care Coordinator will work in partnership with healthcare providers, including maternity and other health specialties, within their local health district and Tiered Perinatal Network, and the Ministry of Health to achieve the aims of Pregnancy Connect.

- What is your understanding of clinical and social needs of pregnant women and their families with complex care requirements?
- Do you have leadership skills and the ability to motivate, inspire and organise staff to achieve organisational outcomes?
- Do you have effective interpersonal skills, including the ability to forge partnerships at all levels both internal and external to the organisation, manage conflict and negotiate resolutions?
