About the RoleWe're seeking a highly skilled Care Coordinator to join our team. As a Care Coordinator, you'll play a vital role in transforming the lives of our clients by connecting them to tailored services and championing their wellbeing.
About the PositionThis is a permanent full-time opportunity that offers a competitive salary with tax benefits and salary packaging options. You'll work from home and on the road, with a company pool car available or reimbursement at $0.96/km when using your own vehicle.Key Responsibilities:

- Craft personalised assessments and goals, measured by outcomes that celebrate strengths.
- Manage budgets and care plans aligned with client choices, ensuring their autonomy and comfort.
- Embrace a dynamic week, traversing your designated region, collaborating with health professionals, and balancing office days from home.
To succeed in this role, you'll need:
- Qualifications in Case Management, Aged Care, or related fields.
- Experience in case management, paired with a knack for tech tools like Microsoft Word, Excel, and Outlook.
- A driver's license, along with the confidence to work autonomously and as part of a team.
