We are seeking a Support Coordinator on a full-time contract till 30 July 2023, with possibility of extension. Someone who values flexibility and wants to be a part of a supportive, inclusive team and work environment.

- Experience working in mental health services

- Demonstrated capacity to work independently and collaboratively within a team environment
- Ability to engage and work alongside clients, service providers and the community that surrounds them

**About the role**

As part of The Way Back Support Service Program, the Support Coordinator provides practical psychosocial support to people experiencing a suicidal crisis or who have recently attempted suicide. Support targets those at the highest risk through referrals following hospital presentations. Throughout the 12-week program, we work with the client to help lift barriers that the client has identified as contributing to their stressors, we also work to empower the client with a voice in their recovery and build knowledge of supports accessible to them when they exit the program. You will make the world of difference to someone at their lowest point in life. This role will bring enormous satisfaction seeing your clients take positive steps forward in their life.

**Duties and responsibilities**
- As a Support Coordinator at Open Minds, you will:_
- Support and guide your clients through a program personalised to meet their individual needs
- Be actively responsible & participate in the collection of data for the purposes of monitoring, reporting, and evaluating
- Build and develop strong relationships with clients, services providers, and health care professionals
- Liaise with hospitals, GPS and health services

**Mandatory Requirements**
- ** **Full driver’s licence
- Covid 19 Vaccinations
- NDIS Workers Screening Clearance (yellow card)
- Working with children check (blue card)
- Tertiary qualifications in Social Work, Human Services, Psychology, Nursing or similar
