Your part in our Blue Care team
The BlueCare Liaison provides leadership, support and guidance onsite and virtually across a Network of local Neighbourhood teams and is responsible for ensuring delivery of quality care, based on assessed clients’ needs that enhances and promotes quality of life.
Key responsibilities include but are limited to:
- Ensuring client care is assessed, planned, implemented, and evaluated in accordance with BlueCare’s Tailor-Made Service Model, as well as funding models, taking into consideration each client’s physical, social, emotional, spiritual, and clinical needs and encouraging client/ client representative participation.
- Complete regular reviews of clinical care/social and wellbeing services ensuring effective scheduling of client services and maximising package spends
- Drive customer experience and customer satisfaction outcomes by acting as the key contact for clients through their BlueCare journey - day to day entry point for service and general and billing enquiries, plan enhancements and changes to funding
- Onboarding and offboarding clients, and management of client information in CRM, ensuring up to date details and case notes.
- Assist with orientation of new staff and ongoing professional development and fostering and driving a high-performance culture and providing great quality of life aligned to the BlueCare values and mission.

What makes a difference for us?
You’ll have demonstrated understanding and sound knowledge of client centred, community-based care service, as well as extensive stakeholder management capabilities and be able to deliver consistently professional and innovative person-centred care.
- Certificate IV in a Health, Community Service or other relevant discipline or equivalent community sector experience.
- Able to satisfy the required probity checks by legislation or Blue Care policies
- Ability to liaise with clinical and care team to provide coordinated care as per the package
- Excellent written and verbal communication skills
- Flexibility to respond to business needs and provide virtual support to leaders and clients across varying locations.
- Demonstrated high-level interpersonal and written communication skills, including intermediate computer skills (e.g. Microsoft Office Suite).
