About the role
The purpose of this role is to provide advanced clinical leadership and clinical expertise relevant to the speciality of Safety and Quality within mental health and alcohol and drug services to support the clinical incident management processes and related patient safety activities across MNMH. The role is responsible for ensuring centralised co-ordination and tracking of very high and extreme risk (SAC 1 and SAC 2 as required) clinical incidents in consultation with the Commissioning Authority (Executive Director MNMH).
Clinical governance of clinical incident management processes, including implementation and monitoring of recommendations and associated clinical service improvements, remains the responsibility of the local mental health and alcohol and drug services.
About you
This successful applicant will carry out the following key accountabilities in accordance with the Metro North Health's values:

- Ensure centralised co-ordination and standardised pathways for clinical incident notification and management across MNMH.
- Co-ordinate and provide technical expertise for the multi-disciplinary review and analysis of very high and extreme risk clinical incidents and sentinel events using the Queensland Health Incident Management Matrix and Root Cause Analysis (RCA) methodology or Human Error and Patient Safety Principles.
- Attend the four (4) facility-based Mortality and Morbidity (M & M) Committees in a patient safety expert role to support consistency in process and documentation of outcomes.
- Coordinate reporting of sentinel events and/or Chief Psychiatrist requirements as relevant.
- Responsible for monitoring all Coronial requests for information, autopsy reports and coronial findings.
- Contribute to MH Death Reviews as required/requested and assist with identification and analysis of other potential issues.
- Maintain a MNMH register of all incident reviews and recommendations, ensuring regular monitoring and review occurs at the MNMH Safety and Quality Committee.
- Communicate on patient safety issues and develop strategies to disseminate 'lessons learned' from RCA and other incident analysis /reviews.
