**Our Journey**:
Social Workers at Monash Children’s Hospital sit within the Monash Children’s Allied Health Team, providing specialised services to infant, paediatric and adolescent patients and their families. Social work services are provided in inpatient settings including Paediatric ICU, Children’s Cancer Centre, Neonatal Intensive Care Unit; in ambulatory care services such as the Department of Paediatric Endocrinology and Diabetes and via state-wide services such as the Paediatric Palliative Care Program and the Victorian Forensic Paediatric Medical Service.

**A Day in the Life**:
This Social Work role sits within the Monash Children’s Allied Health department and has close ties to the Paediatric Diabetes and Endocrinology Department. The position primarily involves supporting children and families with diabetes and other endocrine conditions, however may provide cover to other paediatric caseloads. For the consideration of a full time role, the caseload will include some work within our Adolescent team.

As the Grade 2 Social Worker, you will collaborate with colleagues across the acute and community sectors to foster clinical excellence and consistency of practice within the service. You will provide social work services within outpatient clinics, on inpatient wards and other areas as appropriate. You will be an important member of the Paediatric Endocrinology and Diabetes team. There is scope to develop or use your quality improvement, teaching and/or research skills

You will be required to participate in two Monday evening clinics per month, and the on call after hours social work service across the Monash Hospital sites and may be expected to work across sites within Monash Health as required.

**What you will bring**:

- You will have a good knowledge of resources and understanding of the assessment and intervention frameworks used to guide practice including the Children’s Youth and Families Act and appropriate referral networks.
- You will be able to demonstrate skills in psycho-social assessments, problem solving, communication and discharge planning.
- You will be skilled at supporting families adjusting to their child’s illness and able to provide grief and bereavement support as required.
- You will develop and maintain strong links with universities and your Monash Health social work colleagues and supervise students and other staff as required.
- You will be a dynamic, innovative and flexible practitioner and will receive regular supervision from highly experienced social work supervisors with opportunities for ongoing professional development.

**What you will need**:

- A relevant social work qualification
- Eligible for AASW membership
- More than 2 years of experience preferably in an acute health care setting and/or with children and families
