- This position will provide the successful applicant opportunities to gain experience working in a well-supported multi-disciplinary team environment with a wide variety of acute surgical and medical patients. This role involves the provision of Social Work service, supervision and education of student and junior Social Workers and will be a member of our supportive team. The Social Work team consists of over 40 staff members who work across the City and Royal Park Campuses in acute, sub-acute and community based services across a range of specialties.**Your new role**:

- Complete psychosocial assessments and provide emotional support and counselling to patients and their families and/or carers.
- Liaise with service providers and refer to Community Services, Aged Care Assessment Service and Transition Care Program.
- You will help us achieve excellence in patient care, helping patients in their homes.

**About you**:

- You are a qualified Social Worker (either current member or current eligibility for AASW) with at least 4 years work experience as a Social Worker.
- You are an adaptable quick learner with proven ability to work autonomously in hospital environments.
- You are able to deliver excellent consumer focused clinical care to patients and their carers.
- You have the experience and skills to mentor and supervise junior staff.
