**ABOUT US**

Amana Living is one of the largest Aged Care providers in Western Australia and has been responding to the needs of older people and those who care for them since 1962. We offer a broad range of services, including residential care facilities, transition care programs, retirement living villages, home care, day care, respite, and dementia specific services. Together, we enable older people to maintain their individuality, providing what is needed to support a fulfilling life.

**THE ROLE**

Reporting to the Manager Reablement, the Coordinator Reablement will coordinate and deliver Reablement Services for clients assessed for this program; ensuring excellent communication is provided to other teams, including Customer Service Centre, Clinical Administrators (CA), Resource Coordinators (RC) and members of the multi-disciplinary team (MDT) including the clients GP. The Coordinator Reablement will undertake liaison with referral sources (ACAT teams) and endeavour to accept all referrals for the Reablement programs; completing initial assessments, supporting the client to identify “Goals of Care” and developing care plans and therapeutic services to meet these goals.

**_ Key responsibilities_**
- Respond to and triage enquires received for the Reablement programs.
- Conduct Holistic Nursing Assessments that identify the client’s care needs and goals of care, providing effective personal and nursing care that is best practice, tailored to individual needs to optimise health and wellbeing, meeting the emotional, spiritual and psychological wellbeing needs of clients.
- Work in partnership with MDT to provide reablement services ensuring appropriate care planning, reviews and regularity of scheduled services are implemented to meet client needs.
- Identify and manage risk to provide effective service delivery.
- Treat clients and their families with dignity and respect while maintaining their privacy.
- Complete documentation and reporting as per the Short-Term Restorative Care and Transitional Care guidelines, in the Client Management System (Comcare) during and after service, and spending requests and approvals in Epicor.

**ABOUT YOU**

**_ Essential Criteria_**
- Demonstrated ability to conduct clinical assessments, prepare care plans and deliver clinical care to the older client
- Proven understanding of the aging process and diseases that affect older people
- Experience delivering care within the home environment
- Knowledge of Aged Care Quality Standards and continuous improvement
- Previous experience working with people with dementia
- Superior organisational and time management skills
- Ability to work independently and as a member of a team
- Strong verbal, written and interpersonal skills
- Adept in MS Office (Outlook, Word, Excel and PowerPoint) and client management systems
- Current AHPRA registration
- Evidence of COVID-19 and 2023 Flu vaccinations
- Ability to obtain a National Police Clearance _(within 6-months validity)_
- Successful completion of pre-employment physical and reference checks
- Class C drivers’ licence and use of a personal vehicle for work
- Infection Control training / assessment
- Manual Handling training / assessment
