**About the Client**:
A not-for-profit community-managed mental health service and NDIS service provider who has provided support to people with mental illness and psychiatric disabilities for 36 years across Melbourne’s South-East. They provide a range of services including one on one in-home outreach, care coordination, group activities, family support and more.

**About the Role**:
As the Team Leader of the Support Coordination team, you will be responsible for providing ongoing supervision, development, and support of the team members as well as holding a small caseload of clients yourself. Based in the Mornington office, you will work Monday-Friday from 9 am-5 pm.

**Skills and Abilities**:

- Qualification Requirements - A diploma in Mental Health/Community Services or a Bachelor's in a health-related field is also highly regarded.
- Minimum of 2 years post-graduate experience working within community mental health and/or NDIS psychosocial supports.
- Must be able to provide NDIS Worker Screening, WWCC, Police Check and current Victorian Driver’s Licence.
- Leadership skills including providing supervision, support and guiding a team of staff

**Duties and Responsibilities**:

- Supervise the NDIS Support Coordination program including both individual and team development.
- Daily monitoring of billable hours and preparation of reports.
- Maintain ongoing support and monitoring of client funds as per their NDIS plans.
- Provide supervision and support to team members, promoting participation in all elements of the NDIS system.
- Provide support coordination to a small caseload using a recovery-based approach.
