The Salvation Army Youth Services offer an integrated suite of programs engaging with young people across Australia. Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment, and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.
About the role
We are seeking a dedicated Case Manager for Youth Services to deliver a high-quality case managed accommodation support service across Frankston and Mornington Peninsula regions to young people who are at risk of, or experiencing, homelessness - including to families with accompanying children. You will provide integrated pathways of support for young people’s physical, emotional, social, spiritual and psychological development. This role is instrumental in meeting the emotional and psychological needs of young people in order to assist in creating intentional avenues for young people to explore opportunities, foster hope and bring about sustainable interpersonal change that can enable them to reach their full potential.
You will successfully
- Individual case plans are developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
- Clients are assisted to access support services and establish community supports, and where needed are assisted through advocacy with service providers
- Progress and outcomes of individual case plans are regularly monitored and reviewed and where needed action is taken to ensure case plans continue to meet clients’ needs or goals
- Accurate, up to date client records and data is maintained for all clients in accordance with TSA standards and procedures using TSA information systems
- Risks to clients, staff and TSA are identified and bought to the attention of the Coordinator/Manager as soon as possible
- Service gaps are identified and recommendations for service delivery improvements are made to line-management
This is a permanent full-time position, based in Frankston, VIC. Salary and conditions are in accordance with SCHADS Level 5.
You will have
- A degree qualification in social work, social sciences or related field is preferred
- Experience and competency in delivering case management practice within a homeless service setting
- Well-written verbal and IT communication skills
- Excellent organisation and administrative skills with capacity to balance workload, determine priorities and meet deadlines while also managing the needs of client crisis assistance requirements
- A national criminal history check is required prior to commencement
- Victoria Working with Children check is required prior to commencement
- A current Victorian Drivers Licence
- All mandatory training including TSA specific training is completed and up to date
