**About the role**

The Mater Young Adult Support Unit (YASU) currently has an opportunity available for an experienced, passionate, and motivated **Social Worker** to join the team. This position will be offered on a permanent part time basis (0.6 - 0.8FTE).

The YASU is a specialist multidisciplinary team who adopt a developmentally sensitive, trauma informed and systemic approach to supporting adolescents and young adults with chronic medical conditions. Chronic medical conditions can impact a person’s physical, psychological, social and occupational wellbeing. Young people with chronic conditions often face more difficulties negotiating the tasks of adolescence and young adulthood than their healthy peers. Adolescents and young adults are distinct population with unique needs as they transition out of child/adolescent focused systems and into adult focused systems. As such, YASU provides integrated and holistic strengths-based assessment and evidence-based treatment to help this cohort develop, thrive, prosper and flourish.

**Salary range: $33.99 - $51.37 (Health Practitioner Level 3) per hour plus superannuation and access to not-for-profit Salary Packaging**

**What you’ll be doing**

We are looking for a dynamic, vibrant social worker to join our team. Your duties will include:

- Comprehensive psychosocial assessments of patients, including families, carers and significant others
- Comprehensive interventions in relation to chronic health conditions (with a particular focus on self-management issues, impact on health outcomes and where comorbid psychosocial complexities exist)
- Risk assessments, and comprehensive interventions, for child abuse and neglect, family violence, intimate partner violence
- Capacity, functioning and development assessment, including support and participation requirements, housing and accommodation
- Counselling and targeted therapeutic interventions in order to address adjustment to diagnosis, possible role changes, emotional/social responses to illness and treatment
- Group work and support programs including psycho-education
- Supporting individuals, families and carers to be self-advocates, or advocating on their behalf
- Service coordination and the making of referrals to other agencies (such as Centrelink and Department of Housing)
- Resourcing and support in accessing appropriate information and financial assistance
- Educating hospital staff on patient psychosocial context and needs including the creation of resources for the medical team about common issues such as Disability Support Pension
- Case management for a small number of medically and psychosocially complex young people

**About you**

As the successful applicant, you will have:

- A tertiary degree in Social Work
- A Working with Children Check (Blue Card)
- A passion for working with adolescences and young adults
- Demonstrated knowledge, expertise and skill in the provision of social work interventions for adolescents and young adults
- Proven experience and ability to work effectively in a multi-disciplinary team, establish priorities, exercise initiative, and deliver outputs within agreed timeframes
- Strong communication and interpersonal skills with a proven ability and experience in establishing and developing key relationships with stakeholders
- Demonstrated experience and commitment to professional development and quality improvement and /or research in psychology
- Demonstrated commitment to work, health and safety and positive patient experience
