We are on the lookout for a Support Coordinator to join our team at Disability Support Angels.

At Disability Support Angels, we support clients with complex mental health needs. We are committed to employing individuals who are dedicated to making a difference, have a keen interest in mental health, think innovatively, and can forge enduring connections with clients, their families, and other service providers.

As part of our team, you will have the flexibility to work from home or the office, as we offer a hybrid job model on a full-time basis.

Your responsibilities will include setting up service agreements, conducting plan reviews, performing risk assessments, attending care team meetings, and most importantly, providing essential support when challenges arise.

You will have the freedom to plan your day, with a focus on delivering exceptional support to our clients, enabling you to naturally meet your support hour targets.

Key duties will involve providing Support Coordination and other necessary supports to clients with an NDIS Individualised Funding Package. You will need to maintain a solid understanding of the NDIS and its impact on our clients, continuously improve processes for supporting them through NDIS procedures, and build genuine relationships with each participant. You will also assist participants in understanding and utilizing their NDIS plans, connect them with suitable providers and necessary services, help them develop their skills and independence, and liaise with participants and stakeholders to achieve the best outcomes for them.

We are seeking an experienced support coordinator with a background in working with individuals with disabilities and a strong knowledge of disability rights and services.

The salary for this position is dependent on experience and qualifications, and an immediate start is available.

If you have a practical understanding and knowledge of the NDIS, including funding categories, allocations, processes, regulations, and reviews, along with a minimum of 1-year experience as a Support Coordinator and an NDIS Worker Screening Check, we encourage you to apply.
