If so, we invite you to join our team as an NDIS Support Coordinator. In this role, you will have the opportunity to work closely with participants to understand their unique needs, navigate the NDIS process, and connect them with tailored supports and services.

**Key Responsibilities**:

- **Personalized Support**:

- Conduct comprehensive assessments to identify participants' goals, strengths, and support needs.
- Collaborate with participants to develop individualized support plans that reflect their aspirations and preferences.
- Provide ongoing guidance and advocacy to ensure participants receive the right support at the right time.
- **NDIS Navigation**:

- Guide participants through the NDIS journey, from plan development to implementation and review.
- Assist participants in understanding their NDIS plans, budgets, and funding options.
- Liaise with the NDIA and other stakeholders to address any concerns or challenges participants may encounter.
- **Service Coordination**:

- Identify and connect participants with a wide range of supports and services, including healthcare providers, therapists, and community resources.
- Coordinate service delivery to ensure participants receive timely and effective support.
- Monitor participants' progress and adjust support plans as needed to achieve their goals.
- **Documentation and Reporting**:

- Maintain accurate records of participant interactions, support plans, and service provision.
- Prepare clear and concise reports for participants, families, and funding bodies in compliance with NDIS guidelines.
- Ensure confidentiality and data security in all documentation and communication.
- **Team Collaboration**:

- Work collaboratively with our interdisciplinary team to provide holistic support to participants.
- Participate in team meetings, case conferences, and professional development activities to enhance service delivery and share best practices.

**Qualifications and Experience**:

- Bachelor's degree in social work, psychology, disability studies, or a related field.
- Previous experience in disability support coordination, case management, or a similar role.
- Knowledge of the NDIS framework, policies, and procedures.
- Excellent communication, interpersonal, and problem-solving skills.
- Strong organizational abilities and attention to detail.
- Valid driver's license and access to a reliable vehicle.

- 100 Points of identification.
- A valid Driver's license and comprehensively insured motor vehicle.
- Valid NDIS Worker Screening Check.
- Working With Children's Check.
- Full Australian working rights.

**Job Type**: Casual

Pay: From $32.00 per hour

Expected hours: 15 - 40 per week
