Disability Support Coordinator

We are seeking an exceptionally skilled NDIS Support Coordinator to join our team. This is an exciting opportunity for an individual with a strong dedication to enhancing the lives of individuals with disabilities. As a NDIS Support Coordinator, you will play a pivotal role in delivering comprehensive support coordination services to our participants. Your key responsibilities will involve providing guidance, assistance, and advocacy to participants as they strategize and pursue their NDIS goals and objectives. Additionally, you will be responsible for cultivating strategic partnerships with various service providers and organizations to ensure participants receive the appropriate support. Effective communication and collaboration with NDIS and other stakeholders will also be essential in ensuring the successful implementation of our participants' plans
**Main Duties / Responsibilities**
- Reviewing participants' NDIS plans and ensure appropriate supports and services are implemented.
- Responding to enquiries from participants in a timely manner.
- Collaborate with participants to develop individualized support plans aligned with their preferences and NDIS guidelines.
- Ensure that all funding management activities are undertaken, and that participants can maximise outcomes outlined in individual plans.
- Provide ongoing support, guidance, and advocacy to participants to help them navigate the NDIS process and access appropriate services.
- Assist participants in identifying and connecting with suitable service providers and community resources.
- Review participant’s progress on an ongoing basis and customize support plans accordingly.
- Engage and collaborate with relevant stakeholders, including NDIS representatives, support workers, healthcare professionals, and community organizations.
- Foster positive relationships and maintain regular contact with participants, their families, and support providers to ensure effective coordination of support services.
- Maintain accurate and up-to-date records of participant interactions, support plans, and progress reports.
- Complete all necessary NDIS documentation and reporting requirements in a timely manner.
- Stay informed about the latest developments, policies, and procedures related to the NDIS.
- Preparing and/or collecting and submitting reports as required for NDIS meetings or providers.
- Engage in ongoing professional development activities to enhance knowledge and skills related to disability support services and the NDIS.
**Qualifications/ Requirements**
- **It is advantageous if you hold**:_
- Qualification in Social Work, Social Science, Health Science, Case Management, Mental Health/Psychology, Disability or other Allied Health qualification.
- Previous experience supporting participants that have mental health, psychosocial or disability support needs in a case management style role.
- Extensive knowledge of disability, mainstream, and community services.
**Experiences**
- Demonstrated experience working with individuals with disabilities and/or in the disability support sector.
- Comprehensive understanding of the NDIS framework, guidelines, and processes.
- Strong assessment and coordination skills, with the ability to develop and implement person-centered support plans.
- Excellent communication and interpersonal skills, including the ability to build rapport and establish effective relationships with participants, families, and stakeholders.
- Exceptional organizational and time management abilities to manage multiple caseloads and meet deadlines.
- Knowledge of relevant legislation, policies, and practices related to disability services and support coordination.
- Ability to work autonomously and collaboratively within a multidisciplinary team.
- Proficient computer skills, including the use of relevant software and database systems.
- Sound verbal, written communication, and negotiation skills
- A current full driver's license
- Understanding and respecting the participant’s privacy and balancing this with information shared with the community and mainstream providers.
- Understand the delineation of roles and responsibilities between the NDIS and Corrective Services, particularly around risk.
- Previous experience in community services, disability services, mental health services, or similar is preferred.
**Salary**: Up to $93,000.00 per year
