I am partnered with a Federal Government Agency, in the heart of Geelong to recruit an APS4 Planner for a 12-month, full-time temporary assignment with the potential to extend.

**Your new role**

As the APS4 Planner, you will be responsible for conducting and planning conversations and risk assessments and making reasonable and necessary decisions.

Responsibilities of the role include but are not limited to:

- Gathering information from participants and planning partners over the phone or in person, to enable high-quality planning and evidence-based decision-making.
- Using a range of policies, practice guidance and legislation to make funding decisions in relation to participant plans to complement informal, community and mainstream supports.
- Working closely with participants and their representatives to identify current and future supports, and informal, mainstream and community support options to achieve plan outcomes.
- Liaising with key stakeholders including Partners in the Community to deliver services to participants.
- Resolving issues and complaints in relation to participant plans.
- Completing administrative and data entry tasks relating to participants.
- Contributing to the achievement of key performance indicators, including planning targets.

**What you'll need to succeed**

To succeed as the APS4 Planner, you will be a forward thinking, motivated, resilient, good at building relationships and want to work in a fast-paced environment, along with:

- Excellent communication skills, both written and verbal
- Demonstrated experience working autonomously and as part of a team
- Previous experience engaging with stakeholders
- Demonstrate time management and attention to detail
- IT proficiency, especially MS Office
- Disability, health or aged care experience is desirable
- Working with Childrens Check or willing to obtain
- Australian Citizenship
