**NDIS Support Coordinator (South West Sydney)**

MK Care is a well-established NDIS service provider located in Liverpool and mainly operating in South West Sydney. We are dedicated to fostering independence, promoting opportunities, and encouraging participation among people with disabilities in the culturally diverse areas we operate in.

**Key Objectives of the Support Coordinator**
- To be responsible for all operational functions of the Person Centred support to Participants, their guardians, and where appropriate family members
- Support with the activities of daily living.
- Safe and welcoming environment for Participants and visitors.
- Assessing the service needs of Participants and linking them to the appropriate resources and providers.
- Developing a resource directory of local social service agencies and providers for easy access.
- To work with the relevant Managers and Service Manager to develop and implement systems, policies and procedures that comply with legal and funding requirements, manage risk and continuously improve service and to ensure consistency in support and processes.

**Duties**
- Develop a detailed understanding about each Participant’s individual support needs and goals as documented on their NDIS plan
- Facilitate increased Participant input into the daily operation of the service
- Work with the Service Manager to manage vacancies and Participant transition
- Provide direct support to Participants as needed
- Be aware of, value and respect the cultural, religious and linguistic background of each Participant and staff member
- Work with the Service Manager to continue the development of service specific systems, processes and procedures and actively participate in on-going review of these documents
- Liaise with local services and support Participants to engage with appropriate day program, health, community and social services
- Work closely with NDIA and other stakeholders as identified
- Work within the framework of the NDIS Quality and Safeguards Framework, Privacy Legislation, and other relevant legislation
- Provide input into the budget planning process and ongoing monitoring of income and expenditure
- Ensure compliance with all legislative and regulatory standards and requirements within the facility
- Participate in relevant team meetings and personal development/ mentor support meetings
- Ensure Participant files are consistently maintained in accordance with procedure.

**Skills & Knowledge**
- Skills and experience in working with adults with a disability using a Participant-centred approach
- Scheduling appointments for Participants, answering phones and tracking Participants' service records.
- Well-developed time management skills and ability to prioritise workload in competing demands
- Responding to complaints and resolving issues or matching Participants with better services.
- Strong interpersonal, assessment and problem solving skills
- Ability to work independently and as part of a team
- Understanding of WHS guidelines and management requirements.

**Essential Qualifications & Experience Required**
- In-depth knowledge of the disability sector including NDIS funding and the NDIS Quality and Safeguarding Framework
- Computer literacy, in particular proficiency with Microsoft programs
- Previous experience in supporting, planning and set up of new services or project managing complex change processes is desirable
- Tertiary qualification in Social Science, Health or other relevant professional field is highly desirable.
- Current NSW driver’s licence.
- All relevant NDIS screening checks

**Salary**: $70,000.00 - $85,000.00 per year

Schedule:

- 8 hour shift

Supplemental pay types:

- Commission

- LIVERPOOL, NSW 2170: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
