**Job Overview**: As a Case Manager at SUPPORT NEST, you will be pivotal in ensuring participants receive the highest quality of care and support. This role involves detailed case management responsibilities, focusing on personalized service planning and risk management.

**Key Responsibilities**:

- **Evaluate and Manage Needs**: Conduct comprehensive assessments to evaluate and manage the ongoing needs of participants, creating personalized service plans.
- **Team Collaboration**: Facilitate and participate in care team meetings with allied health professionals, ensuring a person-centered approach.
- **Participant Integration**: Travel across Victoria to meet potential participants, assess their needs, and manage their integration into our services.
- **Record Maintenance**: Maintain accurate and comprehensive case notes and records, adhering to NDIS standards and organizational policies.
- **Stakeholder Communication**: Build strong relationships and communicate effectively with various stakeholders.
- **Support Identification**: Identify appropriate supports, engage with new and existing support providers, and work towards achieving identified outcomes and goals.
- **Crisis Management**: Address points of crisis, barriers, and participant risks holistically, designing solutions to manage these risks.
- **Policy Adaptation**: Stay informed about new processes and changes in NDIS policies, demonstrating a commitment to continuous professional development.

**Qualifications**:

- **Case Management Experience**: Strong foundation in case management within the NDIS framework, with experience in coordinating community-based supports.
- **Technological Proficiency**: Significant understanding of information technology to manage and report on participant care effectively.
- **Problem-Solving Skills**: A strong solution focus and creative thinking ability.
- **Travel Requirements**: Required travel throughout Victoria with a valid driver’s license.
- **Educational Background**: Preferred background in health or social sciences, particularly with experience in handling complex care needs.

**Experience**:

- Minimum of 2 years of experience working with NDIS.
- Minimum of 2 years of experience in case management.

**License/Certification**:

- Working with Children Check (required)
- Driver License (preferred)
- NDIS worker screening check (required)
- First Aid and CPR License (required)
